Class …………… hereby consent to my child’s visit to SICAS Matrix – DHA Phase 3 (Feb 3 - 5, 2023).

I/we understand that this is designed to provide an opportunity to students to engage in out of class learning
experiences.
As this is a public event and media coverage will be done, students’ photos taken may be used in various
ways, online or in print. This may include, but is not limited to, promotional materials or websites.

 International School Lahore shall in no event be liable for any direct, indirect, incidental, special or
consequential damages, losses and mishaps arising out of or relating to this event.

 I ascertain that my child has no illness, allergies, or disability that may require special attention, other
than those already notified to the school.
I agree to reimburse the following:

 Any emergency medical expense incurred by International School Lahore for my child.
 Any expense incurred by International School Lahore because of my child’s misbehavior during the
trip.
I absolve the school of all responsibilities beyond that of normal supervision and duty of care.
I hereby release International School Lahore, its directors, officers, employees, agents and members from
any liabilities, actions, proceedings, losses, costs, damages, expenses, claims and demands arising out of or
relating to this event.
