1 OPERATION AND MAINTENANCE ACTIVITIES
Henningson, Durham & Richardson Architecture and Engineering, P.C. (HDR) continued the daily
operation and maintenance (O&M) of the Claremont Polychemical Superfund Site Groundwater
Treatment System (GWTS) Operable Unit 5 (OU5) during the month of April. This report covers
the operation and maintenance activities for the system during the period defined as beginning
at 0730 hours, April 1, 2020 through 0730 hours, May 1, 2020. O&M conducted during this
reporting period was guided by the site O&M Manual.

The GWTS – t r e a t m e n t plant, grounds, and well systems - were maintained for the 30 days
in this reporting period during which the treatment system experienced 285 minutes of downtime
due to various process pump failures.

Readings of the key plant process parameters are normally recorded each work day (if the plant is
not occupied, the system is monitored remotely). These readings and the Human Machine
Interface (HMI) flow trend lines are used to monitor the system’s performance and condition.
Selected readings are recorded in the daily database which is an electronic file maintained in the
monthly operating document folder.

The treatment process control and alarm systems are functional. Pressure Filter Feed (PFF) Pump
1, and Air Stripper feed (ASF) P1 are off line with mechanical issues. The recovery well pumps and
the process pumps are operated in the automatic mode and are remotely controlled and monitored.
The pump at RW-4, which failed on 12/31/2019, was replaced and put on line April 7. (See
Attachment 1 at the end of this report for a summary of the installation)

1.1 DAILY OPERATIONS SUMMARY REPORTS


The GWTS’s daily operations and maintenance activities, project tasks, and observations during
this period are briefly described in the Daily Operations Summary Report (DOSR). The DOSR is
based in part on the treatment system’s daily operating worksheets and logs which include:

1.2 SUMMARY OF MAINTENANCE ACTIVITIES


The maintenance of the treatment system, facility, and associated equipment is performed in
accordance with the site O&M Manual.

The maintenance, operation, and inspection of the plant incorporates the equipment manufacturers’
recommendations, operations experience, and good engineering and maintenance practices. A detailed
accounting of the April activities is further provided in the plant operator’s daily log book.

Maintenance and project activities completed during April include:

 Routine and general maintenance tasks conducted at the plant, on the grounds, and
in the well fields.
 Grounds were policed and windblown debris was collected on a frequent basis.
 Basin 33 was inspected and the water level gaged. BSP has started to irrigate the
golf courses. This will be an ongoing task during the watering season.
 The repaired MultiRAE meter was calibrated and checked
 Thepower was disconnected at the RW-4 pump. The pressure switch assembly was
removed, 4/6.
 The valve effluent to plant discharge and influent to B-33 was opened. The valve
vault was checked for water flow before and after the valve opening, 4/6.
 The pump and power cable at RW-4 were changed, the PS assembly was reinstalled
and the power reconnected. The overload was reset and the pump was returned to
service, 4/7.
 The AutoDialer was programed to include HDR Project Manager contact information.
 The process pumps seal drains were cleared as necessary
 The bulbs in the N-door exit sign were replaced, 4/14
 The process equipment function tests were completed, 4/15
 The OU4 site and safety inspections were completed, 4/17
 The CPC site inspection was conducted, 4/17
 The State University of New York Farmingdale wellfield inspection was conducted
4/17
 The port covers on the air stripping tower at OU4 were checked for possible use at
OU5
 A path was cleared to Basin 1, 4/22
 The RW System inspection was completed, 4/22
 The OU5 comprehensive site and safety inspections were completed, 4/23
 The plant floors were cleaned 4/23
 The top of the movable shop bench was replaced, 4/27

 ASF High-High Level Alarm (HHLA) lamp was replaced
 The output from the PFF W-2 relay was moved to the W-1 relay. This stopped the
relay chatter and pump cycling. It is a tempo fix. 4/29
 The communication cable for the control system was moved from the network switch
directly to the HMI computer 4/29
 The wiring connections of the air flow meter were checked, 4/30
 The facility was frequently cleaned and scrubbed with bleach in light of the
Coronavirus concerns.
 The monthly equipment function tests were completed.

2 TECHNICAL SUPPORT ACTIVITIES


2.1 HDR Personnel
 HDR maintained the system throughout the period.
 Technical expertise and guidance was provided from the Mahwah, Newark, and NYC
offices
 4/2, HDR NY-IT assisted with installing EQuIS data processor
 4/6, John Guzewich was in to monitor the entry into the RW-4 vault
 4/13 & 4/23, Ed Brandt was in for shelter when Pall job was halted.
 4/17, NY-IT assisted in restoring the remote access to the process control system.
 4/27, Ian Denholm recoded the PFF pump sequencing controls and advised on the
plant restart of 4/29.

2.2 NYSDEC Personnel, sub‐contractors and other visitors


 4/1, Don Griffing, Environmental Assessment and Remediations (EAR), was in to
check on the pump cable availability
 4/2, Valerie Egan, NCDPW, was in for an update
 4/2, PSEG in at OU4 to read the meters

2.3 Deliveries

3 HEALTH AND SAFETY


Work at the Claremont GWTS OU5 was conducted in accordance with the approved Site Safety
and Health Plan (SSHP). Safety related activities during this period included:

 Daily site safety inspections were completed as part of the routine O&M activities.
 A CSE was made into the RW-4 vault, 4/6 to disconnect power and pressure switch.
This was not a permit required entry, but paperwork filed. LOTO procedures were
followed.

 CSEs were made in to the RW-4 vault, 4/7 to replace the pump.
 NYSDEC and HDR COVID-19 documents were reviewed and posted on 4/13.
Additional documentation was reviewed and posted on 4/15.

There were no other safety issues of note in April.

4 PLANNED ACTIVITIES AND SCHEDULES


The evaluation of the plant operating system and equipment is ongoing by HDR. A list in the
form of corrective actions or maintenance tasks has been generated as is a monthly system
status report. These reports are updated as n ee d ed a nd r e vie we d at least monthly. Both
are electronically filed. The corrective action list is included at the end of the text of this report as
Table 6 – Claremont Corrective Action Summary.

Upcoming tasks include:


 The quarterly process water sampling task is scheduled for May 14
 PFF P1 requires testing and inspection
 PFF float switches are to be replaced

5 MONITORING WELL WATER ELEVATIONS


The monitoring well system’s groundwater level elevation data table was updated in March. This
database is available for review. The next synoptic water level round will be scheduled for June,
2020, after which the table will be updated.

6 TREATMENT SYSTEM FLOWS


The volume of treated water discharged by the treatment plant to the selected recharge basin
is generally determined daily from readings of the plant effluent flow meter output. During the
April period, the HMI readings were recorded. The plant continued to operate in the auto mode.
The treatment system experienced mechanical problems and was shut down for a total of 285
minutes for the pump failures.

The total volume of treated water discharged from 0730 hours on April 1, to 0730 hours on May
1, was approximately 30,198,000 gallons. The data in Table 1 shows selected monthly flows
discharged from the plant.

A graphic representation of the system’s daily plant discharge output is provided in Figure 1 and
the daily plant totalizer readings for April are provided in Table 4, both following the text of this
report.

Under current conditions, the Programmable Logic Controller (PLC) and the control system are
stable and fully functional. Flows from the individual recovery wells are remotely read, transmitted,
and totalized.

During April reporting period, the treated water was discharged directly to Recharge Basin No. 1 on
the landfill property. The valve influent to Basin 33 was partially opened as the Bethpage State
Park’s watering season is starting

The flow summary for the processes can be found in Table 5 at the end of this report.

7 CHEMICAL CONSUMPTION
The hydrochloric acid feed system is currently off line and the system is empty of acid. There are
four drums of virgin acid on site. No acid was consumed in April.

The sodium hydroxide storage system is currently not in use and the system is empty of caustic.
There is no bulk sodium hydroxide on site and no caustic was consumed in April.

The sodium hypochlorite storage system is currently not in use and the system is empty of bleach.
No bulk sodium hypochlorite is stored on site. No sodium hypochlorite was consumed in April.

8 WASTE DISPOSAL
There was no waste removed from the site in April

9 MONTHLY DISCHARGE MONITORING REPORT


The GWTS is operated under an equivalency permit from the NYSDEC. The analytical results
for the April plant discharge samples indicated all analyzed parameters were compliant with
permit limits. These results can be seen in Table 7 following the text of this report.

The plant’s water discharge permit is in the process of being renewed by the NYSDEC.

10 PENDING ISSUES AND CONSIDERATIONS


On Tuesday night 4/28, the plant experienced HHLA conditions in the PFF and ASF wet wells.
This sequentially shut the plant down. The causes may have been storm related, but a PFF pump
tripped off and a second pump failed. The third pump was already out of service. Adjustments
were made and the treatment system was returned to service. However, the pumps started short
cycling due to a recurring problem with the float switches. The replacement of these switches is to
be scheduled. (A summary of this incident can be found at the end of this report in Attachment 2)

Pump 1 of the Plant Discharge system is failing and was taken out of service. An evaluation needs
to be made.

Pump 1 of the Air Stripper Feed system has begun to emit a louder pitched whine. It was taken off
line until it can be evaluated.

The disposition of the fire sprinkler system, fire alarm, and central monitoring systems at OU4 are
awaiting the decision of the NYSDEC.

The backflow preventer device on the 6” water line into OU4 failed its December inspection. The
repair proposal was received in March. The disposition of this repair has yet to be determined.

The repairs to the OU5 fire alarm open loop are to be approved by NYSDEC and scheduled.

The plant lights are kept on overnight because the plant lighting and emergency lighting are wired
to the same circuit breaker (sole switch).

The OU4 plant is offline and its disposition including that of the injection well system is pending.

The plant exhaust system is controlled by the methane monitoring system and needs to be

separated.

Removal and disposal of vapor phase activated carbon at OU4 is to be scheduled.

The status of key aspects of OU4 are as follows:


 The plant heat is on
 The fire alarm panels are off-line and there is no central monitoring
 The facility is secure and physical monitoring continues
 The facility and grounds are not maintained

11 PLANT DOCUMENTS
Procedures and standard forms are written, reviewed, and revised as needed. As-built drawings
are generated and updated as necessary. These April activities included:

 GP-03, AutoDialer procedure was updated to rev. F


 GP-06, MultiRAE procedure was updated to rev. B
 Form-04, Air Monitoring Worksheet was revised to rev. H

12 MONITORING RESULTS
The Claremont Polychemical GWTS is monitored through the analysis of off‐site laboratory
analytical data and on‐site field data.

12.1 Off‐site Analytical Data Results


Monthly PD samples are taken for organic analysis in compliance with the NYSDEC discharge
permit. Quarterly groundwater (GW) samples are taken for organic analysis, and quarterly
process water (PW) samples are taken for organic, inorganic, and generic analysis. The April
sampling activities were:

 The March PD data was processed and submitted


 The April PD samples were collected, processed and documents generated, 4/15. The
samples were packed and shipped, 4/16.
 The PW sample bottles were inventoried and ordered for the May event. The sampling
date set for 5/14
 The February PW data was revised and resubmitted, 4/28
 The April data was processed and submitted, 4/29

12.2 Field Data

The treatment plant effluent is monitored for pH and temperature on a weekly basis in order to
obtain a monthly average in compliance with the NYSDEC discharge permit requirements. These
readings are taken from the plant effluent at a controlled point with a calibrated portable meter.
The plant discharge readings for April can be found below in Table 2.

The NYSDEC discharge permit requires the plant discharge to have an average monthly pH
between 6.5 and 8.5 standard units (su). The results for this month meet this requirement. Data
showing the plant discharge’s monthly average pH trend over several months is provided in Table
8 following the text of this report.

AS Tower Air Monitoring

Using a calibrated PID meter, weekly air monitoring readings are taken from the effluent air
stream of the AS Tower through Port B when the treatment system is online. The April readings
from the AS tower are provided in Table 3.

There were no emissions from the Air Stripping System observed this month. No emissions have
been detected since HDR began operation of the plant in October of 2016.

Other routine data collected in April were:


 The electric and water meter readings were recorded weekly.
 The plant sound levels were recorded bi-weekly.
 The electric and gas meter readings for OU4 were recorded monthly.
 The water levels in Sumps 3 and 4 were monitored.
 The recharge basins were inspected and the water levels noted.
 The differential pressure readings across the AS Tower were recorded bi-weekly.

13 PROCESS ANALYSIS and SYSTEM STATUS
The treatment system is currently operated 24/7 in the automatic mode.

13.1 Extraction (RW) Processes


 The motor controls and systems were inspected
 The pump and power cable at RW-4 were replaced and the system was returned
to service, 4/7.
 The pump system is remotely controlled and monitored, it operates in the Auto
mode. All the pumps are now fully functional (RW-3, RW-4, and RW-5).
 Pump flow readouts are transmitted to the plant and the totalizers for 3, 4, and 5
are fully functional.
 The A/V valve at station 16+57 remains isolated from the transmission line.
 The A/V valve at station 17+10 remains isolated from the transmission line.
 RW-1 and RW-2 are off line and periodically run for PM purposes. Their flow
meters are not transmitting.
 The vault heaters are on and panel heaters are active.

13.2 Air Stripping (AS) Process


 The three AS feed pumps are fully functional but P1 was removed from service until
it can be evaluated. The pumps are operated in the auto mode off the wet well level
switches. The system has been coded to force the periodic shut off of the lead pump.
Pump 1 as the lead pump does not keep up with influent flow and therefore it does
not cycle off. This required occasional manual sequencing of the pump. The
sequencing was recoded to force the change.
 The shaft seals were snugged up. Motors and seals were greased.
 The AS tower main drain valve is not functional (fail open).
 The tower media appears clean as the pressure differential between the top and
bottom ports remains relatively constant. The lower section of media has been visually
inspected. Analysis of the sampling data indicates that iron is being removed from the
system.
 The discharge valve for ASF P1 appears to be frozen in the open position.

13.3 Plant Discharge (PD) Process


 Pump 1 has been taken out of service due to excessive noise and vibration.
 The L2 float switch has failed
 The system operating code was revised to allow for smoother sequencing of the
pumps.
 The shaft seals were tightened and greased. The motors were greased.
 The plant discharge continues to be mainly directed to Recharge Basin No. 1. The
valve influent to Basin 33 was opened slightly. As the Park’s watering season gets
underway, the discharge to basin 33 will be increased.
 The discharge valve for PFF P2 appears to be failing in the open position. The valve
for Pump 3 has failed open.

13.4 Other
 The Auto-dialer is fully functional and was reprogramed with additional contact
numbers.
 The plant’s first light bank is wired to the e-light recharging system, therefore the
circuit must be kept on.
 A leak has developed in the water supply line running through the plant. A
temporary patch was installed. The water service was restored at a lower pressure.
 The fire alarm’s plant loop has an open sensor and is awaiting repairs.

14 GROUNDS
14.1 Plant Perimeter
 General outdoor clean-up tasks are on-going.
 The back lot was cleaned up.
 The Frac tank, which is emptied and cleaned, remains on site.
 The outdoor light timer is not operating. Nine of the outdoor building lights are
currently out. These conditions should not impact safety or security.
 The Town of Oyster Bay (TOB) continues to maintain the grounds along the plant
perimeter.

14.2 Well Field


 Well, well field, and basin inspections continue.
 The well access paths are relatively clear, downed trees and overgrowth are
removed as necessary.

14.3 Other
 The grounds continue to be inspected but not maintained at OU4.
 The Claremont site is relatively secured. There is currently no tenant on the property.

RW-4 Pump Replacement

The pump failed at 18:42 on 12-30-19. Initial electrical tests indicated that the contact block engages
but the pump did not activate. Further electrical testing show that the pump motor was bad and needed
to be replaced.

Monday, April 6, the motor controls were locked and tagged out. A confined space entry was made into
the recovery well vault and the power to the pump was disconnected, the pressure switch assembly
was removed, and the manifold discharge valve was closed.

Tuesday, April 7, the operators and contractors mobilized at the Bethpage State Park, Black Course
east of the 7th fairway at Recovery Well 4.

7:30 HDR arrived on site.

7:50 Boulier Inc. crane arrived on site. (Phil Boulier)

8:00 EAR (NYSDEC ‘on-call’ contractors), arrived on site. (Don Griffing, Bruce Campbell)

8:45 The crane was set at RW-4.

9:50 Access hatch removed from pump shed. The pump discharge pipe was disconnected and
sanitary seal freed. Lift commenced.

11:00 The pump assembly cleared the vault and was set on the ground outside the shed. (Lift ~1300
lbs., 125 feet.) A pin-hole leak was observed in the pipe ~20 feet above the pump. The motor
was removed.

12:00 The old pump was removed and the new pump was installed.

12:10 The new motor was installed and the cabling was connected and sealed.

13:45 The pump assembly was lowered into the well and connected to the manifold.

14:05 The pump power was connected, the pressure switch assembly was installed, and the discharge
valve was opened.

14:10 The system was activated in Manual mode at the pump. The crews started clean up and
demobilization.

14:37 The pump was shut off and put in Auto mode. (~270 gpm on 27 minutes)

14:45 Griffing, Campbell and Boulier left site.

15:00 HDR left site

At treatment plant, the system was put into operation in auto mode.

Plant shutdown incident of 04-28-20

The treatment system experienced problems overnight starting April 28.

23:16 Pressure Filter Feed (PFF) – P2 tripped off
23:45 The PFF wet well went into a HHL alarm condition turning off the Air Stripper Feed
(ASF) pumps. The alarm was silenced. The system operated with one PFF pump, the
wet well was pumped down, turning on the ASF pumps.

00:50 The PFF wet well went in HHLA condition again as the one pump could not keep up.
The alarm was silenced.
At this time the operator attempted to access the control system remotely but had
internet connectivity issues.
01:25 The HMI was remotely accessed and it was determined that one of the PFF pumps had
shut down. To slow the influent flow, one of the recovery wells was shut off – RW-4
(arbitrary).
02:29 The PFF wet well went to HHLA condition again. P3 had failed. The alarm was silenced.
02:54 The ASF wet well went to HHLA condition as the pumps were automatically shut off by
the PFF HHLA condition. The alarm was silenced. At this time the recovery well pumps
automatically shut off. The complete treatment system was offline.

07:28 PFF P2 was reset at the circuit breaker. PFF P3 was reset at the overload relay. After a
programmed delay both pumps started to pump down the wet well.
07:37 When the PFF wet well reached low-low level, the ASF pumps were started in Hand
mode and then put in Auto mode. The Air Stripper Blower came on with the pumps.
07:39 AS the ASF wet well pumped down, the pumps for RW-3 and -5 started in Auto. The
pump for RW-3 then shut-off. It was manually restarted in Auto and remained on-line.
07:41 The pump for RW-4, which was manually shut off previously, would not activate in Auto
mode.
09:14 The overload relay of the motor starter for RW-4 was reset and the pump started in Auto
mode.

The entire system was offline 285 minutes


RW-3 was down 287 minutes
RW-4 was down 407 minutes

The system appeared to be running without problem, and then,

13:49 The PFF wet well went to HHLA condition. PFF P3 tripped off again. The alarm silenced
and the pump reset. Relays in control cabinet began buzzing and cycling as did the
pumps.

13:56 the PFF wet well level float switches were manually manipulated. The buzzing and
cycling stopped.
14:00 the output from the W-2 relay (lag pump off) was moved to the output of the W-1 relay
(all pumps off). The buzzing stopped. The pumps appear stable.

Other observations:
1. The remote access to the HMI was disconnected. It appears that the Netgear switch is
bad. The internet cable was run directly from the router to the HMI computer. The
internet connection was restored.
2. The storms of last night (4/28) may have jolted some control components.
3. Float switch 2 appears to be bad. The HHLA condition in the PFF wet well may have
gotten into the float switch connection box causing it to fail. This needs to be addressed.
It has been proposed to run new conduit from the motor controls into the wet well
through an existing grate port.
4. The new coding for the pump sequencing may have overloaded the pump control after
the 4 hour off time causing the PFF P3 to fail a second time
