INDEMNITY BOND

1. I ____________________________ Father / Mother / Guardian

of___________________________ submit an undertaking that I am sending my ward
voluntarily for the SSB Interview Training Program to ace the SSB interview at AFPA India
Private Limited from 17th December 2023 to 30th December 2023.

2. I fully understand that these sessions will involve outdoor tasks which are injury prone. I
hereby indemnify AFPA India Private Limited for any misfortune/accident/personal injuries
to my son/daughter and take full responsibility for the same.

3. I further understand that in any such eventuality the responsibility of AFPA India Private
Limited is limited to admitting my ward to the nearest healthcare facility and it will be my
responsibility to get any further treatment done and pay for the entire expenses.

4. I fully understand that I will not have any right to claim any compensation from AFPA India
Private Limited.

5. I declare that the decision of sending my ward to AFPA India Private Limited Nagpur is my
own and is with complete understanding of the risks involved.

6. I further declare that my ward does not have any history of mental illness including
fits/seizures/shoulder dislocation or any other acute diseases.

7. I understand that my ward will not be permitted to do high risk individual obstacles and
GOR in these sessions in AFPA if he is found overweight.

8. I further understand that in the event of any misconduct created by my ward in the
academy, he will be immediately terminated from the academy and he will have no option
but to leave.
