Master and Crew extended their fullest support including accompanying the inspector throughout the inspection process.
The vessel was well prepared for the inspection and the Master had arranged for all required documentation to be placed
in the Ship's Office. Opening and closing meetings were held here in the presence of the senior officers. The Master,
officers, and crew of the vessel were co-operative cooperative throughout the inspection and were also receptive to
inspection requirements.

The vessel was ICE Classed 3 for light ice conditions and no additional equipment in compliance with Chapter 12, thus this
variant was not selected for this report.

inspected on 24 August 2023.

Other Inspector Comments: The last internal audit was carried out on 24 August 2023. Nil NC
and 3 observations were raised by the Auditor.

Survey and Repair History

2.8 Has the vessel been enrolled in a Classification Society Condition Assessment programme Y N NS NA

Other Inspector Comments: CAP certification was issued by MSR Condition Assessment
Programme. The following rating was issued:

2.9 Are procedures in place to carry out regular inspections of cargo and ballast tanks, void spaces, Y N NS NA
trunks and cofferdams by the vessel's personnel and are records maintained?
Other Inspector Comments: As per company policy water ballast tanks are to be inspected
within twelve months cycle and cargo tanks within thirty-six months. Ballast tanks were
inspected in November 2023 and cargo tanks No. 1 P was inspected on 10 March 2023 after
tank cleaning.

2.11 If the disposal of engine room oily water or sludge to a cargo or slop tank has taken place, has Y N NS NA
the event been recorded in both Oil Record Books, was the receiving tank free of cargo and
have the transfer arrangements been approved as per IOPP Form B?

2.13 Is the vessel provided with an approved Ballast Water and Sediments Management Plan, are Y N NS NA
records maintained of all ballast water exchanges or treatment operations and are the officers
aware of BWM requirements?

Structure

2.16 If any cargo / ballast tanks, void or hold spaces were sighted from the deck, were they in good Y N NS NA
order, free from oil contamination and could the vessel easily check or sample segregated
ballast prior to deballasting?
Other Inspector Comments: WBT # 3 Port & # 3 Starboard side were sighted from deck level
through the small hatch cover and found all in good condition, coatings were intact with no
traces of oil. Sighting ports or sampling ports were also available on manhole covers of
respective tanks.

Other Inspector Comments: Manning level was meeting/exceeding the requirement as follows:
Deck Officers: Minimum 4 / Actual 4.
Deck Ratings: Minimum 5 / Actual 5.

Additionally, there was 1 Cook, 1 Steward, 1 Electrician, 1 Deck cadet and 1 Engine cadet.

Other Inspector Comments: English was the common working language.

Crew Qualifications

3.5 Does the officers' matrix posted for the vessel on the SIRE website accurately reflect the Y N NS NA
information relating to the officers on board at the time of the inspection?
Other Inspector Comments: It was noted that the Master and Chief Offocer had sign on the
same date. The completion of the previous Captain's contract and an urgent need to appoint a
new Senior Assistant, the Owner has decided to simultaneously replace both. This decision
accepted due to the Master , having previously served on this vessel.

3.7 If the vessel is equipped with an Electronic Chart Display and Information System (ECDIS) have Y N NS NA
the Master and deck officers undertaken both, generic training and type-specific familiarisation
on the system fitted onboard?
Other Inspector Comments: The Master and deck officers were each in possession of
certificates for generic and type specific shore based ECDIS training. Additionally, they had
completed an on board ECDIS familiarization checklist upon joining the vessel.

Drug and Alcohol Policy

3.8 Does the operator have measures in place to prevent Drug and Alcohol abuse in accordance Y N NS NA

Other Inspector Comments: An unannounced shore Drug & Alcohol test was carried out on
board every 12 months and was last done on 2 November 2023 by an independent agency
"ARCHIMED-T". An unannounced on board alcohol test was carried out monthly as per office
instruction and was last done on 9 November 2023. All results were negative.

4.12 Is there an effective Chart and Publication (Paper and Electronic) Management System in place Y N NS NA
and are the deck officer's familiar with the process including the effective management of T
and P notices?
Other Inspector Comments: Charts and Publications outfit management service was provided
by a shore vendor. ENCs on the ECDIS units were updated till NTM week 47/2023. The T& P
corrections were interfaced on the ECDIS units using AIO updates.

4.13 Are deck officers aware of the requirements for managing Navtex and Navarea Warnings and is Y N NS NA
there evidence of an effective system in place to monitor these warnings?
Other Inspector Comments: The Navtex self-test was carried out with a satisfactory printout.
Navtex correction was automatically applied on the ECDIS with a manual counter check carried
out by the 2nd Officer.

4.14 Are Master and deck officer's familiar with the operation of the ECDIS system fitted on board? Y N NS NA
Other Inspector Comments: Two sets of Electronic Chart Display System (ECDIS) as primary and
as back-up means of navigation were fitted on board.

4.19 Is the master and deck officers aware of the requirements for the echo sounder and is there Y N NS NA
evidence that it has been in use as appropriate during the voyage?
Other Inspector Comments: The echo sounder safety depth alarm was tested during this
inspection and found working satisfactory.

5.3 Is the appointed Safety Officer suitably trained, aware of his responsibilities and is there Y N NS NA
evidence to show that the safety officer has been effectively performing duties associated with
this role?
Other Inspector Comments: The Chief Officer was the safety officer and he had attended
appropriate training.

5.7 Are crew members participating in safety meetings and is there evidence of effective Y N NS NA
discussions on safety related issues with shore management feedback?
Other Inspector Comments: The safety meeting was held monthly with all officers and crew
members. The operators comments were available.

5.8 Are the crew aware of the requirements for reporting of accidents, incidents, non-conformities Y N NS NA
and near misses and is there an effective system of reporting and follow up investigation in
place?
Other Inspector Comments: An average of 2 Near Miss reports are submitted monthly by all
ranks and departments.

5.10 Are the crew aware of the requirements to keep external doors, ports and windows closed in Y N NS NA
port and is the accommodation space atmosphere maintained at a slightly higher pressure
than that of the ambient air?
Other Inspector Comments: Only one access door was used as entry and exit from the
accommodation.

Enclosed Space and Pump Room Entry Procedures:

5.20 Are the crew aware of the permanent arrangements provided for lifting an incapacitated Y N NS NA
person from the cargo and, if applicable, the ballast pumproom, including provision of a
suitable stretcher or harness and is the equipment in good order?
Other Inspector Comments: Safety harnesses and stretcher were available to be utilized at the
bottom platform with properly fitted lifting arrangements.

Gas Analysing Equipment

5.23 Does the vessel have appropriate duplicate portable gas detection equipment suitable for the Y N NS NA
cargoes carried, are the officers' familiar with the operation, calibration and is the equipment
being maintained in accordance with manufacturers and industry recommendations?
Other Inspector Comments: Various span gases used for calibration of gas detectors on board
were checked and found valid within expiry dates at the time of inspection.

Hot Work Procedures

5.26 Is gas welding and burning equipment in good order and spare oxygen and acetylene cylinders Y N NS NA
stored apart in a well-ventilated location outside of the accommodation and engine room?
Other Inspector Comments: Oxygen and acetylene regulators with flashback arrestors were
fitted at the storage cylinders outside of the accommodation and at the outlet in the engine
room workshop.

Fire Fighting Equipment

5.37 Are the crew aware of the location and use of the International Shore Connection, is it readily Y N NS NA
available externally, is a fire control plan exhibited within the accommodation, also a copy
available externally and equipment correctly marked on the plan?
Other Inspector Comments: One set of international shore connections was always kept at the
manifold in use.

5.39 Are officers aware of the requirements for testing fixed fire detection and alarm systems and Y N NS NA
are the systems in good order and tested regularly?
Other Inspector Comments: As per records on board, all fire detectors including manual call
points were randomly tested on a weekly basis and all detectors were completed within 3
months period.

5.41 Is the emergency fire pump in full operational condition, starting instructions clearly displayed Y N NS NA
and are officers able to operate the pump?
Other Inspector Comments: Emergency Fire Pump located in the steering gear platform was
started and pressure built up to 6 kg/cm2.

Other Inspector Comments: The ships accommodation ladder was rigged on the port side for
safe access.

Pollution Prevention

6.1 Are the ship's crew familiar with their duties in relation to the Shipboard Oil Pollution Y N NS NA
Emergency Plan (SOPEP) / Shipboard Marine Pollution Emergency Plan (SMPEP), is the plan
maintained updated with emergency contacts readily available?
Other Inspector Comments: The Class approved SOPEP was found updated with the latest
contact list and inventory list, these were available for review.

6.2 Is the ship fitted with a main deck boundary coaming and scupper arrangement that is Y N NS NA
effectively plugged during operations?
Other Inspector Comments: The scuppers were plugged with neoprene type plugs.

6.3 Are means readily available for dealing with small oil or chemical spills? Y N NS NA
Other Inspector Comments: Pneumatic spill pumps were mounted on the port and starboard
side at the after end of the main deck with fixed piping was used for suction and discharge was
connected to ROT.

Cargo Operations and Deck Area Pollution Prevention

6.4 Are Annex 1 and 2 overboard valves and cargo system sea valves suitably secured, thoroughly Y N NS NA
checked closed prior to commencement of cargo transfer and where provided, sea valve-
testing arrangements in order and regularly monitored for leakage?
Other Inspector Comments: Annex 1 overboard valve was found locked, closed, lashed, sealed,
and blanked, with warning notice found posted locally. Annex I and cargo system sea valves
were found shut, lashed, sealed, and blanked.

6.5 If ballast lines pass through cargo and/or Bunker tanks are they tested regularly, and the results Y N NS NA
recorded?

Pump Rooms and Oil Discharge Monitors

6.13 If an ODME is fitted, is it in good order, well maintained and any operational downtime Y N NS NA
recorded in the ORB?
Other Inspector Comments: ODME was regularly tested monthly and was found working
satisfactorily. Entry was logged down in the Oil Record Book.

Venting Arrangements

8.19 Are the officers aware of the primary and secondary cargo tank venting systems and are the Y N NS NA
systems functioning correctly?
Other Inspector Comments: The primary venting system was pressure vacuum valves, and the
secondary venting system was pressure sensors fitted in each cargo tank. Primary means of
venting were provided by independent P/V valves fitted to each cargo oil tank, with a setting of
+1400/-350 mmWG. Pressure sensors (secondary means) were fitted for each cargo tank with
a monitoring system in the cargo control room (set to alarm at 1540 / +100 mmWG). The mast
riser and PV Breaker were connected to the inert gas main line.

8.20 If stop valves are fitted which permit isolation of individual tanks from the common venting Y N NS NA
system, are they provided with positive locking arrangements and are the keys under the
control of the person in overall charge of the cargo transfer?
Other Inspector Comments: Positive locking arrangements was by a special key under Chief
Officer's care.

Inert Gas System

8.22 Was the inert gas system in use and operating satisfactorily at the time of the inspection? Y N NS NA

that the oxygen content was 2.0%.

normal level during the cargo operations.

Single Point Moorings

Inspector Observations: The bow stoppers swl fitted on board were noted 200 tons. This is not
meeting recommendation of minimum 350 tons swl.

Issue is well known to the Office and always taken into account prior fixing voyage order.
Consideration for BOW stopper modification will be given for the next dry-docking.

9.26 If the vessel is equipped for mooring at single point moorings, does it meet the Y N NS NA
recommendations as applicable, contained in Mooring Equipment Guidelines?
Inspector Observations: Bow stopper was fitted in such a way that the direction of line passing
through the bow stopper passing two pedestal rollers before going to the windlass tension
drum was noted more than 90 degrees.
Initial Operator Comments: Root Cause
Ship’s design.
Corrective Action
This vessel's of a standard design and while the layout is not ideal they are able to moor safely
to SPM’s. In addition to mitigate risks involved with mooring operations the vessel has a
schematic mooring arrangements picture posted on the forecastle, showing snap back zones to
remind personnel of dangers associated with parting ropes. Also, as per company procedures,
before every single point mooring operations, a Risk Assessment is required to be carried out by
the vessel in cooperation with office.
Preventive Action

Chapter 10: Engine and Steering Compartments

Policies, Procedures and Documentation

10.1 Are the engineers aware of the procedures for safe operation of the machinery plant including Y N NS NA
their duties and watch standing instructions as per the Company SMS and are these
instructions clearly defined?
Inspector Observations: It was noted when tested that the signal column located in the engine
room workshop and outside of the engine control room at 2 locations, no audible alarms was
heard. Spare part requisition was raised on 24 Nov 2023 and awaiting supply.
Initial Operator Comments: Root Cause.
Equipment failure prior inspection.
Corrective Action.
A formal TRA has been developed and approved by the office.The Company procedure for
vessels not fitted with a fixed system or if the system fails where it is installed was
implemented onboard, proper records kept, since the failure of the system.
Further investigation revealed burnt PC board.Requisition for a new issued.
Problem was rectified upon receipt of spares after inspection.PC board was renewed.System
tested and found operational.
Preventive Action.
Observation is circulated to all Company ships and superintendents for information and
prevention, will be further verified during future visits of Company representatives onboard, all
ships are reminded on the Company TRA procedure.

authorized vessel to be operated in manned mode without going into UMS mode.

fitted?

Other Inspector Comments: The fuel oil sample was sent to the shore laboratory for analysis
after each bunker operation. The last two bunker analyses were checked and found within
acceptable limits. Lubricants for Main Engine, Auxiliary Engines every 3 months. Last done on
15 November 2023 and were found fit for continuous usage. Steering Gear No.1 and No.2,
Stern Tube, Windlass including mooring winches, emergency generator, COPT and valve
remote control system every 6 months, Last done on 15 November 2023 and were found fit for
continuous usage.

10.11 If the vessel is fitted with a class approved Exhaust Gas Cleaning System are the officers well Y N NS NA
familiar with the system and safety requirements and are these documented?

Fire Fighting Equipment

10.19 Are diesel engine fuel delivery pipes adequately jacketed or screened, exhaust lines and hot Y N NS NA

Chapter 11: General Appearance and Condition

Hull, superstructure and external weather decks

11.3 Is the general condition, visual appearance and cleanliness of the weather decks satisfactory Y N NS NA
and are deck working areas clearly identified and provided with non-slip surfaces?
Inspector Observations: It was noted that the near manifold area opposite drip tray on port
and starboard side main deck were with scattered thick rust spots. Cargo tank area 6 Starboard
were also noted with thick rust spots. It was noted that 'U Clamps for main cargo pipelines
were corroded. The cargo valves flanges bolt and nut on the main cargo pipeline at the
midships area between manifold port and starboard side area were heavily corroded.
Initial Operator Comments: Root Cause.
Natural corrosion. Lack of maintenance. There is basically slight surface rust present, mainly
cosmetic issue.
Corrective Action.
The problem is known to the ship’s superintendent, works are ongoing with additional crew
supplied specifically for this purpose.Vessel has sufficient stock of paints and all necessary tools.
We expect to complete maintenance of the surface of this particular area by end of December ,
weather permitting.
Preventive Action.
The observation is circulated to all Company ships and superintendents for information and
prevention, will be further verified during future visits of Company representatives onboard.

11.99 Additional Comments

The hull was clean but not free from marine growth. There was some topside paint coating
damage, however the ship side was observed with rust stains from the scupper discharging
trailing vertically to the water line The superstructure coatings were clean but not free from
rust stains. The weather deck coatings were in satisfactory condition with no significant rust
scale except for some location as mentioned in VIQ 11.3. There was evidence of routine
coating maintenance in place. The overall level of cleanliness, tidiness, sanitation, and hygiene
throughout the accommodation and food areas were noted to be in good condition.
