INSTRUCTIONS

Use SCORECARD worksheet to calculate SCORE and CLASSIFICATION RATING for each
project
Instructions for Design Assessment:
1 Fill in the project name and Registration No. in DESIGN tab for Design Assessment.

2 Select type of project: "NEW ROADS" or "UPGRADING ROADS". Maximum points will be allocated
according to the type of project selected.

3 Insert TARGET point for each criteria (from drop down selection / key in).

4 Core criteria which is not mandatory and not applicable to the project may select "NA" as target point.
Point will be autamatically deducted from the total maximum core point.

5 Mandatory core criteria will not have option to select "NA" as target point. Choose "0" if criteria not
fulfilled.

6 Facilitator need to make sure that target point are selected for all criteria and to make sure that completed
submittal documents are submitted together with this scorecard for assessment purpose.

7 The total points accumulated will be automatically calculated.

8 Point under ASSESSMENT will be given by the panel of pH Assessor according to the submittal
documents.

9 Final Score for the assessment are based on the following formula:
