DATABASE MODEL
Entity-Relationship Diagram
The system flow starts when the admin performs the daily check-up. After all data has been translated
into information and stored in a database, the administrator can access the data stored in the database.
BHW
The Barangay Health Workers (BHW) and barangay clerk can access the entire core modules of
the project.
Can manage the information of the following patients under the barangay health center such as:
record ID, date admitted, date discharged and the treatment of the patient.
Assists the midwife on checking the patient
Check if there are available rooms that the patient can be used
PATIENT RECORDS
This feature is used to manage the information of the following patients under the barangay health
center. The BHW has the access to manage and overview it.
MIDWIFE
assist the BHW from the identification of the health problems and patient medical needs
Checks the patient’s health condition
PATIENT
They can be given medicine based on the prescriptions and diagnosis of the BHW or doctor
They can be diagnosed based on the result of the diagnosis together with the diagnosis ID
They can be assigned to a room when needed
DIAGNOSIS
The result can be presented to the patient, which will help them identify which medicine is
needed
Project Flowchart
The flowchart elaborates each process in the proposed system in a particular and systematic way. The
system starts with the login process for the administrator, which is the Barangay Health Workers (BHW).
They will login with their account or create one if they don’t have one yet. After the admin logs in to
their account, they will manage the patients. If the patient is not yet registered, the admin will add the
patient’s details to be recorded in the database. If they are already registered, the administrator will
manage the test details. If the patient has not been checked or tested by the midwife, they will have to
consult with them. After seeing them or having already been consulted by the midwife, the
administrator will then manage the diagnosis. Lastly, the administrator will update the patient’s records
and then logout.
