Dear Applicant,

We are excited that you are applying to the Jubilee Children’s Fund (JCF) scholarship.
Jubilee Insurance is driven by integrity, passion, leadership, excellence and teamwork.
The Jubilee Children’s Fund (JCF) was established on 10th September 2008 by Jubilee
Insurance Limited which seeks to alleviate the hardships faced by underprivileged
children in education, health and social & cultural issues.

The JCF scholarship 2024 covers Secondary School tuition fees payment for four years
and school uniforms upto form four.

Before you begin filling out the application form, please read the information below
carefully.

QUALIFICATION

i. You must have scored 350 and above

ii. You must be from an economically disadvantaged background (Orphan, guardians that can’t support you, parents with
chronic illness, disabled needy parents e.t.c.)
iii. You must be a Kenyan Citizen
iv. You must have your result slip and a letter of admission or calling letter
v. You must have a fee structure from the admitting school
vi. You must have done your KCPE in a public primary school
vii. You must be admitted to a public secondary school
viii. You must be nominated by a Jubilee Insurance staff member

INSTRUCTIONS/GUIDELINES

The information provided in this form is intended to help the Jubilee Children’s Fund Committee understand the applicant’s academic
and financial position for the purpose of assessment for scholarship/award.

• The application MUST be submitted on email jcf@jubileekenya.com. No hard copy applications will be received.
• This application form MUST be filled accurately.
• Fill the form in capital letters.
• All incomplete or inaccurately filled forms will be automatically rejected.
• Copies of ALL DOCUMENTS required must be provided by the applicant and sent on email. Any applications without relevant
• documents will be rejected.
• The completion and submission of this form is not a guarantee for sponsorship.
• Any false statements, omissions or forged documents will lead to automatic disqualification.
• JCF reserves the right to make the final determination of scholarship beneficiaries.
• Only 2023 KCPE candidates will be considered.

SECTION H: DATA CONSENT

The Jubilee Children Fund was established on 10th September 2008 by Jubilee Holdings with the aim of alleviating the
hardships faced by underprivileged children in their pursuit for education, health and to bridge the social cultural gap. The Fund
enables children from underprivileged backgrounds attain a high school education through sponsorship by Jubilee Holdings.
The children are nominated by staff members and require that the child must have attended a public primary school and scored
350 marks and above, come from an underprivileged home and seeking to join a public secondary school.

The scholarships awarded are then showcased as part of the Corporate Social Responsibility projects carried out by Jubilee
Holdings. Communication on the same is distributed internally to staff who also contribute to the fund, as well as externally to
promote access to education and build awareness of the scholarship and its benefits.

As part of the nomination and evaluation process, Jubilee Holdings collects information about the parent/guardian and the
child. This includes their names, national identity number, residential address, contacts, age, primary school attended, high
school the child is hoping to enrol in, the financial background of the parent/guardian. The personal data obtained is used to
evaluate the worthiness of the nominees and communicate internally and externally about the project.

PERSONAL DATA PROCESSING CONSENT

Without expectation of compensation or other remuneration, now or in the future, I___________________________ [your name

sensitization on its benefits should my child be awarded the scholarship.

I consent to the processing of the personal data of the above-named child and Jubilee Holdings has assured me that the
personal data to be processed will only be used for the purpose of this campaign and will be available on the internal networks
and social media for a period of 6 months after posting, after which the same shall be deleted/removed from these sites.

This consent includes permission to: -

a. Interview, film, photograph, tape, audio or otherwise make a video reproduction of my child and I and/or record my/
her/his voice.
b. Use my child’s name, image, marks, and our background, financial capacity and funding for my child’s education.
c. Use my name, image, voice, and story.
d. Use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s) or reproduction(s) of me, and/
or recording of my voice, in part or in whole, in its internal communication channels, social media applications such as
YouTube, Instagram, Twitter, LinkedIn and Facebook.
e. Retain and use my name, my child’s name, image, marks and our story to raise awareness of the JCF scholarship and to
offer sensitization on its benefits.

I confirm that I am aware that social media networks are public platforms, and the general public will come into possession
of this information and may download, repost and reshare our personal data as disclosed to Jubilee Holdings. I acknowledge
that such actions are beyond the control of Jubilee Holdings, and I will not hold them liable for any further reproduction by the
public. Jubilee Holdings guarantees that it will process my data and the data of my child in a lawful manner and in compliance
with the provisions of the Data Protection Act 2019 and the Data Protection (General) Regulations 2021. Jubilee Holdings
also guarantees that they have put in place appropriate security measures to safeguard our data during processing, storage,
and destruction.

although this is a communication campaign it might enhance the image and reputation of Jubilee Holdings Ltd.
