4. Theoretical Approach
For this counselling session, a humanistic theoretical approach is utilized to further increase
rapport and build a therapeutic relationship with the client. In the future session, a different
approach might be considered to aid the client with her issue.
Presentation
• Client is conflicted and uneasy with brother due to household issues affecting daily life

• Social: Unable to receive direct interaction with closest social support due to living distance

with the client regarding the issue client faces

brother

8. Counsellor’s Reflection
The first session with the client has showcased that the presenting problems do not greatly impact the
client’s daily life. Client’s daily life and autonomy is not interrupted, and client is capable of handling
daily tasks and responsibilities well. As the session went on, it is evident that the client demonstrates to
have above average Emotional Intelligence (EQ) and is capable of understanding her situation and
emotions well. In the following session, it is to observe on client’s progress on handling the issue with
the insight given.

4. Theoretical Approach
For this counselling session, the Person-Centered Theory (PCT) was chosen as the theoretical
approach. Client’s good Emotional Quotient (EQ) Due to the client’s severity of the issue is
not a risk towards the client’s daily autonomy and mental health, certain key concepts of the
Solution Focused Brief Therapy (SFBT) method were chosen to aid the client’s issues. These
concepts are namely, focusing on the future, giving compliments, and inviting clients to do
more of what is working. SFBT’s short term-oriented key concepts are a good fit for the
client’s situation, integrated with PCT.

To supplement the previous insight given to the client by adding another layer of depth to the first
insight.
Additional Information:
Client has shown to successfully adapted to the insight given during the first session, shows good
susceptibility to be able to accept the added layer of depth to the insight.

8. Counsellor’s Reflection
The second session with the client has shown improvements to the client’s situation. Client is shown to
be able to utilize the insight given during the first session and able to adapt to her circumstances. As
forecasted in the prognosis of the 8P, client’s resources with good social support, above average
emotional intelligence (EQ) has proven to be helpful in alleviating her current stress. The session is
considered a success as the client no longer feels any need for future sessions and is shown to be in
good autonomy and well-being. The chosen methods of therapy are well acquitted and a good match for
the client’s level.

Individual Counselling
This informed consent document is intended to provide general information about the counseling services
provided by Management & Science University under-graduate student. This is a legal document; please
read it carefully before signing.
• Nature of Counseling: The type and extent of services that I/my child will receive will be determined
following an initial assessment and through discussion with me. I understand that there may be both benefits
and risks associated with participation in counseling. Counseling may improve ability to relate to others,
provide a clearer understanding of self, values, and goals, and an ability to deal with everyday stress. I
understand that counseling may also lead to unanticipated feelings and change, which might have an
unexpected impact on me/my child and my/my child’s relationships.
• Supervision: I understand that Ahmad Farhan Haikal (full legal name ofcounselor-in-training):
o is currently under training his/her Degree in Counseling and Guidance at Management & Science
University. In order to improve his/her skills, he/she is required to complete a practicum and two
internships.
o is currently under the direction of a site supervisor that is required to have a minimum of a master’s degree;
preferably in a counseling, or a related profession with relevant certifications and/or licenses; a minimum
of two years of pertinent professional experience; knowledge of MSU’s counseling program requirements,
expectations, and evaluation procedures; and relevant counseling supervision training.

faculty supervisor.
• Confidentiality: I understand that counselors maintain confidentiality in accordance with the ethical
guidelines and legal requirements of their profession. Effective counseling, however, sometimes requires
that confidential information be shared with other staff members, professors, or graduate students who are
training at Management & Science University. When confidential information must be shared, pseudonyms
(false names) are used to protect the identity of the client. I understand that no records or information about
me will be released outside Management & Science University without my permission, except under certain
circumstances: if I/my child present/presents a serious danger to self or other person(s); if there is a suspicion
or actual incident of child abuse or neglect; or a valid subpoena is issued for my/my child’s records, or
my/my child’s records are otherwise subject to a court order or other legal process requiring disclosure.

• Video/Audio Recording of Counseling Sessions: I understand that the Management &
Science University counselor-in-training routinely records counseling sessions. I
understand that such recordings will only be used for educational purposes and that the
professors and/or students involved will respect and protect the confidential nature of the
sessions. It is understood that the recordings will be confidential and only reviewed for
supervision or educational purposes and will subsequently be erased by the counselor-in-
training within 90 days or at the end of the semester, whichever comes first. I understand
that because these are digital recordings, confidentiality is limited by the secureness of the
technology being used to store them. All efforts are made to keep recordings confidential
but the possibility of unforeseen technological events mean that confidentiality cannot be
absolutely guaranteed. I understand that all such recordings are property of the Management
& Science University.
If I have any questions regarding this consent form or about the services offered, I
understand that I maydiscuss them with my counselor-in-training or his/her site supervisor.
