Confined Spaces

HIGH RISK MANAGEMENT STANDARD

DO
Check you actually need to enter the confined space

Complete a risk assessment and get a confined space permit authorised before starting work

Ensure that everyone on the job is trained and competent for confined space entry

Ensure there is adequate ventilation (e.g. use a fan if required)

Perform a pre-use inspection and only use PPE and life-saving equipment that is fit for
purpose with a current test certificate (e.g. harness, tripod, gas detector)

Test and monitor the atmosphere before and during entry

Know the triggers to evacuate and have an evacuation and rescue plan in place

Stop the job, evacuate the confined space and reassess if conditions or work STOP
requirements change

DO NOT
Enter or perform work in a confined space unless you are trained, competent and authorised

Work in a confined space without isolating all hazardous energy sources

(e.g. hydraulic, stored, chemical, kinetic and mechanical sources)

Enter a confined space without testing the atmosphere first

Place ventilation equipment where it can draw in contaminated air (e.g. near equipment
exhausts or sources of gases or vapours)

Perform a confined space rescue without testing the atmosphere and using the right equipment
(e.g. breathing apparatus)

Enter a confined space without an authorised observer/stand-by person present

Introduce new hazards (e.g. use of chemicals or hot work) without controls

Return a confined space to service unless everyone is accounted for
