AFFIDAVIT OF GUARDIANSHIP

sworn to in accordance with law, depose and say –

That I am the biological mother and guardian of _________________, a minor

aged _____ years old, born on ____________, and who has an interest in the estate of
his late father, _______________;

That the said minor is under my care and custody;

That I am competent to receive on behalf of the said minor and in minor’s name
his share in the estate of his deceased father, _________________;

That I am not an imbecile, insane, a vagrant, a vicious person or a habitual

drunkard or habitual criminal, and have not abandoned, neglected or refused to support
said minor or caused him to commit offenses against the law;

That I am executing this affidavit to confirm and attest to the truth of the foregoing
facts, for the purpose of receiving on behalf of the said minor and in minor’s name his
share in the estate of his deceased father, and for whatever legal purpose it may serve.
