AFFIDAVIT OF EMPLOYMENT
KNOW ALL MEN BY THESE PRESENTS:

I, RANDY N. BALDERAS, of legal age, single, Filipino citizen and a resident of

Amallapay, Tubao, La Union, after having been duly sworn to in accordance with law, do
hereby depose and state, that:

1. I am employed as STAFF NURSE at TUBAO BIRTHING HOME located

at Poblacion, Tubao, La Union from February 21, 2011 up to present; copy of
my Certification is hereto attached and made an integral party hereof;

2. I have not violated any Foreign or Philippine Law of whatsoever kind and/or
nature;

3. Neither is there any case pending, civil, criminal and/or administrative against
my person in any court or tribunal nor I have been convicted of any crime in
violation of any laws of the Republic of the Philippines;

4. I am executing this Affidavit to attest to the truthfulness and veracity of the

foregoing narration of facts; and for whatever legal intents and purposes this
may well serve.
