IN RE: FINAL DEMAND TO FULFILL YOUR LEGAL OBLIGATION

PURSUANT TO THE WRITTEN AND NOTARIZED AGREEMENT
AND PAY THE SUM OF FIFTY THOUSAND PESOS (Php50,000.00)
REPRESENTING INDEMNITY FOR THE DAMAGES INCURRED
TO THE MOTORCYCLE OWNED BY ROLANDO F. PAYUKET

Greetings:

I am writing you on behalf of my client ROLANDO F. PAYUKET who incurred several bodily
injuries and damage to his property (i.e. motorcycle bearing Plate No./MV File No. 0401-
1620422) as a consequence of having been struck down by the passenger bus (i.e. bearing Plate
No. DYA-707 and registered in the name of JAM LINER INC.) driven by JONALD O. SINAG
while on duty as passenger bus driver for JAM LINER INC.

In a nutshell, the parties agreed to settle this matter amicably wherein said JONALD O. SINAG
undertook and committed to settle and pay ANY AND ALL expenses necessary and incurred
for the treatment of the wounds sustained by ROLANDO F. PAYUKET and for then repair and
restoration of his motorcycle; hollow promises which until present date remains
UNFULFILLED albeit these guarantees were reduced in writing and made under oath.

I am now therefore giving you a period of no more than THREE (3) DAYS of receipt of this
notice to FULFILL YOUR LEGAL OBLIGATION PURSUANT TO THE WRITTEN
AND NOTARIZED AGREEMENT AND PAY THE SUM OF FIFTY THOUSAND
PESOS (Php50,000.00) REPRESENTING INDEMNITY FOR THE DAMAGES
INCURRED TO THE MOTORCYCLE owned by my client ROLANDO F.
PAYUKET, otherwise I will be compelled to file the corresponding criminal and civil cases to
protect the rights and interest of my client.
