For each of the following determine which cranial nerve is being evaluated.
1. Patient is experiences loss of balance, dizziness and nausea. CN VIII
2. Patient has diminished saliva. CN VII & CN IX 3. Patient cannot bring up the corners of their mouth to smile. CN VII 4. Patient suffers from anosmia (loss of smell). CN I 5. Patient is deaf in one ear. CN VIII 6. Ask a patient to tell you the number of fingers you are holding up. CN II (vision), students may add CN 7, 9, 10, 12 to include speech component 7. You savor the flavor of your morning coffee. CN VII & CN IX 8. Patient has difficulty swallowing. CN IX & X 9. Tickle the nostrils with a feather invoking a sneeze. CN V 10. Patient has difficulty chewing on the left side of their face following a stroke. CN V 11. Patient cannot rotate their eye down and out. CN IV 12. Patient cannot protrude their tongue. CN XII 13. Your tongue is painful after burning it drinking a hot beverage. CN V 14. The eye of a patient with nerve damage deviates medially. CN VI 15. A stroke results in loss of tear production. CN VII 16. Patient lower eyelid droops preventing the eyelids from completely closing. CNVII 17. Patient has vocal cord paralysis. CN X 18. Patient fails to detect the bitter taste of quinine when placed on the back of the tongue. CN IX 19. Check the gag reflex by touching a tongue depressor to the back of the throat. CN V (sensory), IX & X (motor) 20. Patient has difficulty raising their right shoulder or turning their head towards the right side. CN XI 21. Patient has a droopy upper eyelid and the eye is deviated laterally. In the same eye the pupil fails to constrict when a light is shined into the eye. CN III
