your good Office the intention to enroll in the Electronic Filing and Payment Systems (EFPS).

The purpose of the enrolment is

Our Company is a Licensed Local contractor or is participating in public bidding
Our Company is identified as a TAMP taxpayer.
Our Company has a complete Computerized Accounting System.
Our Company is an Accredited/Prospective Importer
Our organization is a National Government Agency/Corporation
Our Company has a paid-up Capital of P10M and above
Our Company enjoys Fiscal Incentives.
Our Company would like to Volunteer to utilize the EFPS

Our company intends also to enroll in the electronic payment system of EFPS-Accredited Agent Bank
Metrobank immediately or within a week after ‘On-line enrolment’, and Activation by the BIR-CSS
Chief. Submitted herewith are the Secretary’s Certificate, and Board Resolution authorizing our
assigned personnel to undertake E-filing and/or E-payment transactions; the updated Certificate of
Registration (COR- BIR Form 2303) which includes Form/s ______________ as our additional tax
return compliance and/or BIR Form 1903 as required therein.
