AFFIDAVIT

I, Mr. Dilip Dolui, son of Late Netai Dolui, by Nationality Indian, religion Hindu, residing at, 17/1, Bama
Charan Roy Road, Police Station Behala, District South 24 Parganas, Kolkata-700 034, West Bengal, am
representing her daughter Ms. Subhashri Dolui, aged about 15(fifteen) years, by Nationality Indian,
religion Hindu, residing at, 17/1, Bama Charan Roy Road, Police Station Behala, District South 24
Parganas, Kolkata-700 034 do hereby solemnly affirm, declare and state as follows :-

1. That in my daughter’s Birth Certificate duly issued by Kolkata Municipal Corporation vide Sl. No.
0224122 dated 8th April, 2009 it was mentioned that her name is Subashri Dolui whereas in her
Aadhaar Card being No. 2426 8748 4196 duly issued by The Government of India her name is
Subhashri Dolui.

2. That now I intend to keep her name as “Subhashri Dolui” in place of ‘Subashri Dolui” due to actual
spelling of her name.

3. That ‘Subhashri Dolui and “Subashri Dolui” identically refer to her and is one and same identical
person, not are two different persons.

4. That henceforth she shall be known, called and recognized as only “Subhashri Dolui” on the
strength of this affidavit.

5. That I am a citizen of India by birth.

That the abovementioned statements made by me are true to the best my knowledge and belief and I
sign this affidavit on ___________day of September, 2023, at Kolkata.
