the participation of my son ____________________________during the 2023 Council Wide Scouts
Jamboree from November 3, 4 5 and 10, 11,12 two weekends to avoid disruptions of classes within
the 5 day school days at Mobud, Punta Blanca, Oroquieta City.

I/We have considered the values and skills that my son will gain from his participation in this activity
provided that due care and precaution will be observed to ensure the comfort and safety of my son
and that DepEd employees and personnel may not be held responsible for any untoward incident
that may happen beyond their control.
