Thank you for uploading your uniform measurement’s to the uniform provider’s online self-
ordering portal.
Our uniform supplier works with an external company to produce the badges you will need to
wear when attending your practice placements. I am therefore seeking your permission to
release your home address details to the uniform supplier to pass on to the badge supplier.
Please only provide address below if it is different to the one we have listed for you on our
university database. You can only use your first and surname as we have it on our database,
preferred or nicknames are not permitted.
