    from August 20, 2023 up to present and medication was taken that caused the improvement of the head ache,
    also the following laboratories specifically the CT scan was also negative and normal.

    This is to certify that HILLARY ANN VALIENTE, 19 y/o female of Pasig City had a teleconsult with Dr.Carol T.
    Laguiwed-Aplat on August 29, 2023 with the following diagnosis:

    “IRON-DEFICIENCY ANEMIA”

    REMARKS:

    The patient was advised the following:

    - Eat iron-rich foods


    - Aim to get seven to eight hours of sleep
    - Take vitamins with iron
    - FIT TO WORK and may go back to on August 30, 2023
