report from THEFT INCIDENT to CARNAPPING because the person involved in the incident report filed
last March 06, 2022, her address indicated in the Barangay Certificate is no longer her address and
cannot be reached/contacted anymore.

Therefore, I am appointing Jacquiline Asne as my authorized representative to perform the following

actions on my behalf:

-Provide all necessary information and details required to complete the police report accurately.

-Obtain a copy of the filed police report and any associated documents.

I understand that Ms.Jacqueline Asne will act responsibly and in good faith while handling this matter..

behalf in this matter.
