    requirements for taking the exam and observe proper health protocol.

    Requirements:
    1. PHOTOCOPY of the following:
    A. 2x2 ID picture
    B. PSA (Philippine Statistics Authority) Authenticated Birth Certificate
    C. Form 138/ or TOR/ or Certificate of SHS Enrollment/ or ALS Cert with score.
    2. Printed copy of this email.
    3. Vaccination Card.
    4. Government recognized ID or High School ID.
    5. Ballpen and Mongol 2 Pencil
