I, MYBELLE AMARRO SANTOS, of legal age, Filipino citizen, married, and a resident of Blk
103 Lot 38 Golden Horizon, Brgy. Hugo Perez, Trece Martires City, Cavite, Philippines, after having
sworn to in accordance with law, hereby depose and state:

1. That I am the mother of MAYTHA AMARRO SANTOS, who was born on August 28,
2013 at Pasay City, Metro Manila;

2. That I have personal knowledge to declare that my daughter was not baptized in any
Religious Sect;

3. That because of this circumstance no baptismal certificate can be presented for

purposes of corroboration ;

4. That I am executing this affidavit to attest to the truthfulness of the foregoing

declarations under penalties of perjury, moreover to serve other legal intents.
