In pursuit of our academic endeavors, I am writing to formally request permission to conduct a survey
using the Technology Integration Evaluation Tool, which aims to assess the extent to which
technology is integrated into Instructional Materials and Educational Resources. This is part of our
coursework for PCK 5 – Building and Enhancing New Literacies Across the Curriculum.

Through the guidance of our esteemed instructor, Dr. Joselin S. Alvarez, our survey’s objective is to
gain valuable insights into the current state of technology integration within the school, specifically
focusing on the use of digital tools in crafting and providing Instructional Material and Educational
Resources. The survey will involve gathering feedback from 30 teachers across different departments.
The insights gained from this survey will contribute to identifying areas for improvement and guide
initiatives to enhance teaching strategies through technology integration.

We kindly request your permission to proceed with this survey, and we seek your guidance on any
specific procedures or requirements necessary for its implementation. We understand the importance
