Parent’s Consent

Ito ang patunay na pinapayagan ko ang aking anak na si Jennilyn P.

Montemayor para sa kanyang Student Internship Program (SIP) sa loob ng
600 oras o 15 linggo simula February hanggang May, 2024 sa DCFE
Corporation bilang requirement sa kanyang pag-aaral sa kursong Bachelor of
Science in Business Administration Major in Marketing Management.

Ito rin ay patunay na ang aking anak ay susunod sa mga batas at

regulasyon na ibibigay ng kanyang paglilingkurang HTE para sa kanyang
ikabubuti at kaligtasan.

Sumasang-ayon ako na may responsibilidad ako, Sherlyn P.

Montemayor, ang Bulacan State University (HEI), ang DCFE Corporation
(H.T.E), at iba pang representante kung sakaling may mangyaring
insidente sa aking anak sa oras ng kanyang Student Internship Program
(SIP).
