AFFIDAVIT OF DISCREPANCY

I, CELSO JAGAPE MOSQUITO, Filipino, widower, and a resident of

Butuan City, after having been duly sworn to in accordance with law, do hereby
depose and state THAT:

1. I am the father of JOCELYN GURREA MOSQUITO, who was born

on December 21, 1969.
2. There is a DISCREPANCY in her Birth Certificate, in which the
“DATE OF MARRIAGE OF PARENTS” was erroneously entered
therein as “NOVEMBER 22, 1967”, when in truth and in fact it should
be “NOVEMBER 15, 1967”, as evidenced by my fourth child's birth
certificate, RONALD GURREA MOSQUITO.
3. For all intents and purposes, the dates “NOVEMBER 22, 1967” and
“NOVEMBER 15, 1967” refer to same date.
4. I am executing this affidavit to attest to the veracity of the foregoing for
whatever legal purpose it may serve.
