1. Appearance
The patient is a 12-year-old female, she was properly dressed, kept, simple & neat. She was
wearing pieces of jewelry on her wrists and she has nail polish.
Hair appears to be medium straight and brown in color.
There are no distinguishing features such as tattoos and scars.
She remains eye to eye contact, she was comfortable with the person and she is paying attention.

The client seems very happy and attentive during the interview because she was singing and
humming.
She sings and dances during the interview.
The patient wasn’t irritable or anxious.

The client’s facial expression aligned with the patient’s emotions consistently.

I think she has delusions of grandeur because she believes that she knows everything than her
teachers.
She thinks that she is above everything.
She believes that she has power when she closes her eyes, she can read people’s thoughts.
