Assess the following areas:

1. Appearance

The patient is a 12-year-old female, she was properly dressed, kept, simple & neat. She was
wearing pieces of jewelry on her wrists and she has nail polish.
Hair appears to be medium straight and brown in color.
There are no distinguishing features such as tattoos and scars.

2. Behavior (motor activity, mannerisms, gestures body language, etc.)

She remains eye to eye contact, she was comfortable with the person and she is paying attention.

3. Speech

The client speaks fast but in a way of understandable manner.


She was attentive, cooperative and participative during the interview.
She has the ability to comprehend language, uses correct words but she answers questions she
thought that she is correct.

4. Mood

The client seems very happy and attentive during the interview because she was singing and
humming.
She sings and dances during the interview.
The patient wasn’t irritable or anxious.

5. Affect

The client’s facial expression aligned with the patient’s emotions consistently.

7. Thoughts

I think she has delusions of grandeur because she believes that she knows everything than her
teachers.
She thinks that she is above everything.
She believes that she has power when she closes her eyes, she can read people’s thoughts.

8. Perception

The client experiencing delusions of grandeur because she believes that she knows everything
than her teachers.

9. Insight

The patient is not aware and doesn’t understand her problem.

10. Judgment

The patient is still young and maybe she will not be able to understand her problem but through
the help and guidance of her psychiatrist and family, she will be able to cope up with her
condition.
