1. I am an applicant for Hazardous Waste Generator

Registrationpursuant to DENR Administrative Order No. 22,.Series
of 2013 of RA 6969;

2. I am aware and understand all duties, responsibilities and

accountability to be imposed upon me and Manango Hospital by
such application and as such will be liable for aU damages incurred
and for any untoward incident during its actual operation;

3. I attest to the truth, accuracy and genuineness of all

theinformation, documents and records contained and attached
tothis application and that I and Manango Hospital shall be liable
for anymisrepresentation, fraudulent declaration and all
itsconsequences;

4. I am executing this affidavit as a proof of good faith in complying

with the requirements for securing the abovementioned
