The Annual Physical Examination (P.E.) of Teaching and Non-Teaching personnel will commence on February 1, 2023 and shall end on June 14, 2024. 2. It is informed that the minimum required laboratory requirements for the Annual Physical Examination are the following: ‘Laboratory Results Validity Urinalysis Within one (1) month from the time of examination Complete Blood Within one (1) month from the time of Count (CBC) _| examination Chest X-ray Within six (6) months from the time of examination | Note: Pregnant women are required to submit Sputum Analysis in lieu of the Standard Chest X-ray. 3. Teachers’ Annual Physical Examination shall be based on the filled-out Form 86 (Health Examination Record) and Oral Health Examination Record for ‘Teaching and Non-Teaching Personnel in accordance to Dep. Ed. Memorandum No. 22 s. 2015. Said forms will be reproduced thru the School MOOE of Elementary and Secondary Schools. Please see attached Form 86 and Oral Health Examination Record to be used marked as Annex A. 4. Oral Examination by the Division Dentist to all Teaching and Non-Teaching Personnel will be a requirement for the Annual Dental Clearance. 5. All Division Dentists, District and Secondary Nurses that will conduct the Annual PE. Annual PE shall only be done in the Schools or District Offices. Public Schools District Supervisors and School Administrators are advised to coordinate with your respective Division Dentists, District / Cluster Nurses for proper scheduling. 7. Only Department of Health (DOH) licensed clinical Mobile Laboratories shall be allowed to conduct remote collection facilities in schools. Mobile Laboratories should follow the DOH Department Circular No. 2018-0223 - Guidelines in Securing Remote Collection Permit for Clinical Laboratories. Schools shall coordinate with their respective District and Cluster Nurses to ensure the legitimacy of Mobile Laboratories and the Strict Minimum Health Standards are implemented during the conduct of Laboratory Examination. 8. Furthermore, all Division Dentists, District and Cluster Nurses should submit their 2024 Annual Physical Examination Consolidated Report on or before June 28, 2023 to the Schools Division Office, School Health Section in Hard and Soft copy through bernadette.carilhayO01@deped.gov.ph for Dentists and nursesreports.davaocitw@gmail.com for Nurses. 9.
