BEFORE YOU BEGIN Please read the following guidelines before starting your aplication. “The application consists ofS sections. Completion ofall section s mandatory before submission. In adaiton to the application you will ned to provide the fllowing documents: + Allcancidates must held current membership prior to application + Acopy of your CV + You job description ‘+ An organisation chare showing your postion, name, whe you are responsible for and accountable to + Your CPD record for the past [2 months (nly required ifyou have been in membership for one year or more) “+ Your forward action plan for the next year (ths should include personal and company objectives), Please note: we can only process appliations which include all f the above.
