Read the case notes and complete the writing task which follows:
Notes:
Hospital: Bloombay Hospital
Patient name: Agnes Moore
Age: 53
Height: 5 feet 2 inches
Weight: 140lbs
Social history:
● Husband retired from the services
● One daughter-Married and settled in London
Medical history:
● Hypertension and diabetes-diagnosed May 1993
● Peripheral artery disease of the legs-December 2003
● Left foot turns out on ambulation-husband stated she has weak ankle and chronic
burning pain in it
Admitted: 2nd April 2011 with breathing problems.
● BP noted as 170/110
● Lisinopril was given
● Condition noted as stable (regular check-ups recommended)
Tests conducted:
● Urine test
● Blood test (normal)
Medical course: Recommended the same prescription that the patient was using for
hypertension/diabetes
Writing task:
Using the information in the case notes, write a letter to Dr Ferret Meynell, 38 Pacific
Highway, St Leonards, NSW stating the condition of the patient in detail.
In your answer:
● Expand the relevant notes into complete sentences
● Do not use not form
● Use letter format
● The body of the letter should be between 180 and 200 words
