RE : Procedures for Clearance Due to Non-compliance with the Teacher’s


Behavior Inventory (TBI) of the 1st Semester of SY 2023-2024

1. First-Time Non-Compliance with TBI Requirement

Students failing to meet the TBI requirement for the first time will retain their block assignment
and will be automatically enrolled upon payment of the required down payment within the
prescribed period. However, the concerned students must personally submit a signed written
apology addressed to Mr. Benjamin Felix Urbano, Jr., Head of OSA-Discipline Office, as endorsed
by their respective chairpersons for clearance purposes on or before January 31, 2024. Students of
the College of Architecture, Pharmacy, and Nursing must have their written apology signed by
their respective Deans.

2. Second-Time Non-Compliance with TBI Requirement

Students failing to comply with the TBI requirement for the second time will be excluded from
their block assignment, and their enrollment will be placed on hold until a clearance is obtained
from OSA-Discipline. Concerned students must personally submit a written apology to OSA-
Discipline, addressed to Benjamin Felix A. Urbano, Jr. Head, OSA-Discipline Office, as endorsed
by their respective chairpersons. However, students of the College of Architecture, Pharmacy, and
Nursing must have their written apology signed by the Dean. In addition, such students must also
render twenty (20), hours of Community Service.
3. Third-Time Non-Compliance with TBI Requirement

Students failing to meet the TBI requirement for the third time or more will be excluded from their
block assignment and their enrollment be placed on hold until the clearance is obtained from OSA-
Discipline. Concerned students must personally submit their written apology to OSA-Discipline,
addressed to Benjamin Felix A. Urbano, Jr., as endorsed by their respective chairperson. However,
students of the College of Architecture, Pharmacy, and Nursing must have their written apology
signed by the Dean Such students are required to complete forty (40) hours of community service.
An additional twenty (20) hours of Community Service shall be imposed for non-compliance
beyond the third offense.

Please note that the stated sanctions do not apply to students of SVST, Graduate School, and the
College of Law.

All letters must be computerized and personally submitted to the OSA-Discipline Section for
approval.
