The questionnaire should be filled by Managing Director in collaboration
with Head of HRM department
2. PURPOSE OF THE QUESTIONNAIRE
The purpose of the questionnaire is to collect information on the organizational
set up of MDAs focusing on their organizational structures, and staffing
requirements.

1. TITLE OF INSTRUMENT: Profile of Staff and Identification of Training Needs


2. PURPOSE

The purpose of this instrument is to obtain the profile of each of the staff member within the department. The information required
includes name of the employee, sex, date of birth, academic qualification, and years with the Agency, employee’s department/ division,
current position and grade, payroll and ID number or passport number, current annual salary and first priority future training needs.
