Bachuma Primary Hospital MDT Round Protocol/2016 E.

This protocol devised for maximizing quality of care of the patients by Multi-Disciplinary Team
(MDT) undergoing the round and it should be governing as well as practical in each wards.

1. The clinical round should fulfill the required multidisciplinary team composition (physician,
IESO, nurse/midwife, clinical pharmacy, laboratory personnel and others).

2. It should be led by the existing senior professionals

3. The required pertinent evaluation and focused discussion should be done by the team to make as
patient centered as possible which includes:

 Prompt patent evaluations and outcomes


 Discussion of scope of investigation
 Relevant chart reviewing
 Pharmaceutical monitors
 Patient evolvement (CRC)

4. Transcription of patient’s information over round book done by the nurses.

All the round equipment with round table (glove, antiseptic agent, medication, procedure materials
and other relevant materials) should always available.

6. Rounds should be conducted regularly and started every morning from 2:45 – 3:00 unless
otherwise clearly stated it must not be skipped, if skipped report to case team focal accordingly.

7. No one is remain absent from his/her assigned ward during round.

8. Regular nursing round is done every 8 hours and handover should be booked properly.

9. The senior professionals should ascertain participation of the student.

10. The MDT should certified for his/her conducting round with the name & profession alongside
his/her signature over the round log book and properly submitted to case team focal.
