JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MYRA AMOZON, of legal age, Filipino, and presently residing at

Urban Deca Homes Banilad, Mandaue City Cebu, and JESSIE RHEY
SARAGENA, of legal age, Filipino, and presently living at Zone Sayote Paknaan
Mandaue City, both after being duly sworn to in accordance with the law, depose
and state:

1. That we personally know of IAN JOHN EMPENADO AGUELL, being an

acquaintance of his for a long period of time;
2. That we declare, confirm, and affirm that he was born at San Antonio
Street, Mandaue City, Province of Cebu, Philippines, on the 31st of October
in the year 1990;
3. That he has been using the said name and birth date in all his documents
and transactions;
4. For the purposes of his transaction with Social Security Systems, we state,
declare, and confirm that “IAN JOHN EMPENADO AGUELA” and “IAN
JOHN EMPENADO AGUELL” refer to one and the same person;
5. That we are not related by affinity or consanguinity to the
above-mentioned person; and
6. That we are executing this affidavit to confirm and affirm the foregoing
facts and for whatever legal purpose it may serve.
