Waiver for PROCARE Camp 2023 (Minor Participants)

I, ______________________, the parent or legal guardian of _________________________,

hereby acknowledge that I have been informed of the details of the conduct of the Face-to-Face
Provincial College Admission Review and Career Orientation (PROCARE Camp 2023), organized by the
University of the Philippines Kalipunan ng mga Anak ng Isabela (UP KAISA).

I understand that UP KAISA shall implement the minimum safety and public health standards set
by the government (such as requiring attendees to be fully vaccinated, filling out health declaration form,
and more) to minimize risk of the spread of COVID-19 or any other risks that might arise therein, but it
cannot guarantee that my child will not become infected with COVID-19 or exposed to any other risks
such as, but not limited to, injuries.

I understand that the organizers of PROCARE Camp mandated that my child shall follow the
health protocols of Cauayan City, Isabela, and the additional precautions set by the event organizers.

I understand that my child’s in-person attendance in the venue, the Cauayan City Stand-Alone
Senior High School, will include associating with facilitators, fellow learners and school personnels, and
other persons inside and outside of the school that may put my child at risk of, but not limited to, COVID-
19 transmission, notwithstanding the precautions undertaken by the organizers.

To the extent allowed by law and rules, I hereby agree to waive, release, and discharge any and
all claims, causes of action, damages, and rights against UP KAISA, PROCARE Camp organizers, its
sponsors and other parties associated.

With full understanding, I – on behalf of myself, my household members, and my child – hereby
freely and voluntarily give my consent to my child’s participation in the activity.
