Please note that this only shows the payments you've made up to the date of this letter.
Payments are recorded against the actual tax year in which they were received, and not
the date they were due. HMRC do have time limits for making a backdated tax relief
claim. This is generally four years prior to the current tax year, but can depend on your
personal circumstances.

Don't forget, there are deadlines for getting your claim to HMRC. Detailed information
on making a claim is available from HMRC on 0300 200 3300 or online at
https://www.hmrc.gov.uk/incometax/how-to-get.htm.

You can download a tax relief form from the RCN website at
https://www.rcn.org.uk/membership/tax-relief. Please send your claim to HMRC and not
to the RCN. Their address is PAYE AND SELF ASSESSMENT, HMRC, BX9 1AS.

As claims for tax relief are assessed on an individual basis, any queries about your
