Patient’s name, age, specific reason for the visit Came to the ER after a fall in the grocery store. X-rays have confirmed
a R humeral fracture.

BACKGROUND She has been palced in a triangular sling while she awaits an
orthopedic consult. She has been prescribed Torodol 10 to 15 mg
Patient’s primary diagnosis, date of IM/IV q4 to 6 hours prn as well as Actaminophen 300mg + codine 30
admission, current orders for patient mg 1 to 2 tabs PO q 4 to 6 hours prn. Neuro vascular assessment and
CSMT are to be conducted every hour. She may have a diet as
tolerated.

ASSESSMENT She has been given medication that does not seem to be effective as
she still rates her pain 8/10. Her Neuro vascular and CSMT assessment
Current pertinent assessment data using head shows she has normal colour, cap. Refill is less then 3 seconds, she has
to toe approach, pertinent diagnostics, vital a decreased RoM in her R should, her temp is stable when compared to
the other arm, she has movement in her R fingers, wrist and elbow,
signs
slight swelling in her R shoulder and minimal deformity.

RECOMMENDATION Perhaphs she needs a different pain medication the help manage her
pain.
