REQUIRES

Single Foreign Trade Application (SUCE) www.vuce.gob.pe. To Obtain SUCE Number you must
process it with your Bank Payment Code (CPB), this request is an Affidavit and includes the
following information:

a.1) Name or company name, address and Single Taxpayer Registry number of the natural or
legal person requesting registration or re-registration.

a.2) Name that reflects the true nature of the product and brand of the product.

a.3) Name or company name, address and country of the manufacturing establishment.

a.4) Results of physical-chemical and microbiological analysis of the finished product,


processed and issued by the factory's quality control laboratory or by an INACAL accredited
laboratory or other accrediting body from a foreign country that has international recognition
as a signatory of the Agreement. Mutual Recognition from ILAC (International Laboratory
Accreditation Cooperation) or IAAC (Inter American Accreditation Cooperation). More

a.5) Result of Bromatological Analysis processed and issued by a laboratory accredited by the
National Institute of Quality - INACAL or another accrediting body from a foreign country that
has international recognition, signatory of the Mutual Recognition Agreement of ILAC
(International Laboratory Accreditation Cooperation) or of the IAAC (Inter American
Accreditation Cooperation), for Foods for special diets, which must indicate their nutritional
properties.

a.6) List of ingredients and quantitative composition of the additives, identifying the latter by
their generic name and their international numerical reference. (SIN code)

a.7) Conservation and storage conditions.

a.8) Data on the packaging used, considering type, material and presentations.

a.9) Useful life period of the product under normal conservation and storage conditions.

a.10) Production Lot identification system.

a.11) Affidavit of compliance with the labeling content established in article 117 of DS N° 007-

d) Name and address of the importer, what may appear on the additional label
e) Number of the sanitary register
f) Due date – when the product require it with accordance with what is established
codex alimentarius
g) Code or batch key
h) special conservation conditions when the product requires it
