Diagnosis of ADHD
4. Atleast one face to face encounter with the child and parent/guardian will be required prior to
initiating treatment for ADHD. Both parents andior all guardians are encouraged to atten this
visit.
2. Vanderbilt Behavior rating scales (both parent and teacher) will be used to help confirm the
diagnosis of ADHD.
3. We may refer the child to a psychologist or psychiatrist to aid in the diagnosis (and to perhaps aid
in the management) of ADHD and/or other coexisting conditions in some cases.
Initial treatment of ADHD
4. The treatment of choice for children with ADHD (in addition to behavioral strategies) is
medication, typically a stimulant medication
2. Finding the most effective medical treatment for ADHD may require several changes in both
dosage and type of medication
Follow up and management of patients with ADHD
4. ADHDis a chronic condition that requires periodic contact between the farnily and physician to
ensure effective management. To conform with national guidelines and to ensure appropriate
and optimal care for our patients with ADHD, we require to see our patient with ADHD every 3-6
months. Yournext appointment will need to be scheduled before leaving our office.

3. One visit, each year will be an annual well child visit. The remaining visits will be med check
visits, which will be every three to six months.
4. For each visit, the patient will need to bring Vanderbilt evaluation forms from the parents and
teachers in addition to the child's report card. These forms will be provided at each appointment
or may be found on our website.
5. In addition to reviewing the evaluation forms and discussing the patient's progress and attainment
of goals, the child’s vital signs, growth, and development will be assessed at each visit.
6. You are encouraged to schedule follow up medication check appointments in advance. Please
plan accordingly as we will not work in medication check appointments if your child is out of
medication.
7. ADHD medications will not be refilled unless follow-up criteria are met.
ADHD Medication Refills
1. Please send a portal message to your Primary Doctor's MA to request a refill when the patient
has 5-7 doses remaining. All prescriptions require 24-48 hours to process from the initial
request. Requests must be made through the patient portal and not through the pharmacy.
2. The State of Texas requires controlled medications be sent to the pharmacy by Electronic
Prescribing (E-Scribe). This decreases the chances of error allowing accurate prescribing

2. Does this eliminate the need for scheduled med checks?
> No. By law, we are required to monitor patients receiving controlled medications. The
patient is required to be seen every 3-6 months at the PCP’s discretion. Factors that
may influence the length between checkups are the type of medication, possible side
effects, and overall well-being of the patient.
3. Can| call the pharmacy to request the refill?
> No. Please make the request to your PCP’s MA through the portal
4. How long before the prescription is ready to be picked up at the pharmacy?
> Check with your pharmacy 24-48 hours after your medication refill request has been
made to the office
5. Can my PCP refill a controlled medication that has been prescribed by another
provider? Ex. Psychiatrist, neurologist, etc.
