I/We hereby willingly and voluntarily give consent to the participation of my/our son,

sponsored by the Sangguniang Kabataan.
I/We have considered the benefits that my son will derive from his participation in this
activity provided that due care, diligence and necessary precautions will be observed to ensure his
health and safety.
In this connection, I/we also volunteer to provide to my/our son the materials and other
preparations required in the said event.
Further, I/We authorize the personnel of Sangguniang Kabataan of Barangay Quezon,
Solano, Nueva Vizcaya to collect, process, retain, and dispose of personal information of the above-
mentioned athlete in accordance with the Data Privacy Act of 2012.
