CASE STUDY ACTIVITY


1. A 65-year-old woman, a smoker, was anxiously brought by his spouse to the emergency
department with a complaint of 3 days progressive weakness. She denies chest pain
radiating to his left arm. But admits to shortness of breath that increases with activity.
She has also had epigastric pain with nausea that has been intermittent for 20-30
minutes over the last three days. Upon assesment, the patient has been taking ACE
inhibitor, antihyperlipidemic, salicylate, platelet aggregation inhibitor, and biguanide
antidiabetic. Undenyingly states that, She has smoked 1 pack per day the past 40
years.
2. VS: Her pulse rate is 128, Respiratory rate is 24 and 108/58 blood pressure and body
temperature is 37.3 with oxygen saturation of 99% and the pain is 5/10.
3. Global left ventricle hypokinesis with ejection fraction of 25% (ECG) and Scattered
bilateral opacities consistent with atelectasis or pulmonary edema are seen in radiologic
report (XRAY).
4. Basic Metabolic Panel (BMP)
Sodium : 135 mEq/L
Potassium: 4.1 mEq/L
Glucose: 184 mEq/L
Creatinine: 1.5 mEq/L
Magnesium: 1.8 mEq/L
5. Cardiac Labs:
