    Two (2) Copies of Duly filled-out/ Handwritten BFP Applicant Information Form;
    B. Two (2) Copies of Duly filled-out/ Handwritten Personal Data Sheet (PDS) CSC Form 212 revised
    2017. Print entries using BLACK BALLPEN (BLUE INK for signature). Do not use sign pen;
    C. One (1) authenticated copy of Transcript of Records and College Diploma (Authenticated by School
    Registrar);
    D. One (1) authenticated copy of Second Level Eligibility from CSC (Professional/FOE)/PRC/MARINA
    (Board of Rating);
    E. One (1) Authenticated copy of PRC ID from PRC (if PRC eligible);
    F. Original Copy of Certificate of Live birth in Security Paper issued by PSA;
    G. Original Copy of Certificate of Marriage issued by NSO/PSA (for married female applicants only);
    H. Original/authenticated Copy of the following clearances;
    a. Police clearance
    b. NBI clearance
    I. Authenticated Photocopy of the following;
    a. Tax Identification (TIN) ID and Certification
    b. Philhealth Card and MDR
    c. Pag-Ibig Fund Card and MDF
