AFFIDAVIT OF LOSS

I, JESKA MANUELLE ZOLETA, single, of legal age, Filipino citizen with

residential address at Tower 1, Unit 1501 Avida Prime Towers Taft, Taft Ave.,
Brgy 36. Pasay City, Metro Manila after having sworn to in accordance with
the law hereby depose and state, That:

1. I am a high school alumna of Maryhill College, Lucena City with address

at M. L. Tagarao Street, Lucena, 4301 Quezon;

2. I graduated on the said institution on March 29, 2011 and as such was
presented with a High School Diploma;

3. I kept the same within a document folder and left inside my cabinet for
safekeeping;

4. Sometime on January 09, 2024, as I was clearing up my cabinet, I

stumbled upon my document folder containing my personal documents,
I was shocked to find that my high school diploma was no longer inside
the document folder;

5. I looked around my cabinet, bags and my surrounding but the same

could no longer be found and is considered lost beyond recovery;

6. I could not recall the circumstance as to how my high school diploma got
lost and the loss was not intentional and was not made for any unlawful
purpose.

7. I am executing this affidavit freely and voluntary to attest to the truth of

the foregoing and for the issuance of a replacement of High School
Diploma in lieu of the lost one from the Maryhill College, Lucena City.
