RESOLUTION AUTHORIZING THE UTILIZATION OF THE EXISTING CHILD

DEVELOPMENT CENTER FOR THE IMPLEMENTATION OF THE PROPOSED SUB-
PROJECT _________________________ TO BE FUNDED BY KALAHI-CIDSS PMNP

WHEREAS, the Barangay Grijalvo of the Municipality of San Fernando Camarines Sur is a recipient of
Kapit-Bisig Laban sa Kahirapan Comprehensive and Integrated Delivery of Social Services Philippine
Multisectoral Nutrition Project (KALAHI-CIDSS PMNP) of Department of Social Welfare and
Development;

WHEREAS, Barangay Grijalvo is expressing our unending support to the program to be able to address the
pressing situation of the community by providing a convergent effort with other NGA’s and the Barangay
Local Government Unit;

WHEREAS, Barangay Grijalvo conducted its BNC MEETING held last ___________ at the Barangay
Hall, and upon discussion and deliberation it was unanimously approved to authorized the utilization of
Barangay owned lot at ________________________________________ of this Barangay for the
_____________________________ with ___________________ physical target;

WHEREAS, Barangay Grijalvo, has an existing Child Development Center which is owned and managed
by the Barangay Local Government Unit of Grijalvo, San Fernando Camarines Sur;

NOW THEREFORE on the motion presented by ________________ and seconded by everyone present,
be it RESOLVED as it is hereby resolved AUTHORIZE THE UTILIZATION OF THE EXISTING CHILD
DEVELOPMENT CENTER FOR THE SUBPROJECT ________________________________________
FUNDED BY DSWD KALAHI-CIDSS PMNP;

RESOLVED FURTHER, that copies of this Resolution be furnished to the Municipal Local Government
Unit through KALAHI-CIDSS PMNP Office, the Department of Social Welfare and Development and
Municipal Disaster Risk Reduction and Management Council for their information and reference;

RESOLVED FINALLY, that copy of this resolution is furnished to all concerned.
