AFFIDAVIT OF PERSONAL SERVICE

I, AAA, Filipino, of legal age, with postal address at Heyrosa and Associates Law
Offices, Grnd Floor, Rebea Bldg., S.B. Cabahug St., corner P. Gomez St., Brgy. Centro, Mandaue
City, after being duly sworn according to law, hereby depose and state that:

1. I am the clerk/ messenger of Law Offices;

2. On January 19, 2024, my services were engaged to serve a demand letter dated 04
December 2023 to SPOUSES BBB AND CCC at their residence located at Zenia Purok 2, Sun-ok,
Tayud, Consolacion, Cebu;
3. On the said date, I personally met SPOUSES BBB AND CCC at their afore-mentioned
resident address;
4. At that instance, I served to them a copy of the Demand Letter dated 04 December
2023;
5. They personally received a copy of the same Demand Letter, however, they
manifested to me that they were previously warned by their lawyer not to sign any proof of
receipt of any demand letter that may be sent to them;
6. I am executing this Affidavit to show proof that a copy a Demand Letter dated 04
December 2023, was received by SPOUSES BBB AND CCC for them to pay the sum of EIGHTY
SIX THOUSAND PESOS (Php 86,000.00) Philippine currency to DDD.

IN WITNESS WHEREOF, I have hereunto set my hand this 22 nd day of January 2024 at
the City of Mandaue, Philippines.
