Department on Aging Membership
To be eligible to receive services through the Geauga Department
on Aging, individuals must meet the following criteria: Be a
Geauga County Resident, age 60 years or older, Register annually
with the Geauga County Department on Aging, Meet program
eligibility criteria, and pay program co-pays/fees, if applicable,

Social Services
Consultation and Care Coordination
e Social Services Program provides professional social work
support for eligible seniors and/or their family. The program is
centered on assisting seniors to remain independent or in the
least restrictive setting as possible.
Assistance with Daily Living (ADL) Program
is program provides non-medical in home assistance for
gible seniors. Its purpose is to assist seniors to achieve
ptimal functioning in their home.
Home Delivered Meals
M Program provides eligible seniors a nutritionally balanced
lunch and a friendly volunteer for a face to face visit up to five

Congregate Meals
ie Senior Lunch Program offers a nutritionally balanced meal
at our Senior Centers up to five days per week.
Home Safety
is pragram is designed to provide health, safety, fire prevention
and ADA (Americans with Disabilities Act) adaptations to homes.
Out of County Medical Transport
is program provides Out of County Transportation to medical
appointments outside Geauga County (designated zones of
surrounding counties)

Adult Day Services (A.D.S.) to Geauga County Seniors who are
in need of a caring, positive and structured environment during
the day due to memory loss, cognitive impairment or Dementia,
Donated Medical Equipment Program
This program loans gently used durable medical equipment
to seniors.

The Transportation Voucher Program provides eligible seniors
with a Geauga Transit Voucher that can be used for doctor
appointments and/or grocery shopping within Geauga County,
