AUTHORITY TO DEDUCT

A Yubikey has issued to me so that I can have access to our client tools. I hereby acknowledge that the
said Yubikey is part of my responsibility and it costs PHP 2,500.00.

As such, I am authorizing Payroll to deduct the said amount from my salary based on the following
schedule in case of loss:

PHP1000.00
Yubikey 2500 PHP1000.00
PHP500.00

It is understood that upon my separation from the employment for whatever reason, the whole balance
of my aforesaid accountability shall automatically become due and payable. SITEL is hereby authorized
to apply any, and all benefits me firstly, in payment of whatever amount due and payable to SITEL by
reason of the above-mentioned loss of the Yubikey.

This authorization shall be valid and binding until my above-stated obligation is paid in full.
