2.02 the social contract

Today, it is important to study and know about social contract. in other


word, what does the society expected from medial professions and doctor.
So, we asked four persons multiple questions that each one of them have
different aspect as

1. Medicine today is caring and compassionate.


Aim/Topic of discussion: All management is aimed at 'cure' and as early a
hospital discharge as possible.

1 is totally agree.
“Lots of time I visit an emergence and they just give me a medication
and don’t take my case seriously” he said

3 disagree.
“I disagree. I had an surgery before, the doctor refusal to discharge after
the surgery because I need to be under observation for some afew days”
he said

2. Medicine today is self-regulating in the sense that it ensures and


maintains standards of professionalism and competence.

Aim/Topic of discussion all doctors protect each other and that the so-called
'self-regulation from the medical profession' is inadequate and
covers up mistakes rather than discuss them openly.

“I have raised a compline about one doctor and no one of medical stuff
responded to me or take it seriously”

“I have seen a meeting between a doctors in the hospital where they discuss
about the cases and how to mange it so they correct each other”

Modern doctors are trustworthy and do not pursue self-interest but have
the patient as their first priority.

All of them agree.


“one time I have an emergency that need a special doctor so they call him to
came and treat me” he said
4. The public should have a more significant say in Medical Disciplinary
Boards.

3 agree “always onesome one try to diagnose himself and not seek for medical
help that’s will led to misdiagnose and maybe complications”

One disagree. “I can see some people in social media have more knowledge
than doctor do”

5. The cost of health care is not the concern of the individual doctor.
Doctors should only focus on their own patient’s best interest.

3 agree “I think doctors should always focus in best choices of treatment to


ensure the best prognosis and not make the patients feel that he is like
commodity”

One disagree. He said” it should take into consideration the cost of


consultation room. That is because not everyone can afford the cost of
expensive treatment and there is always another choice which is cheaper.”

6. The free market-system has become too dominant in modern health care
and its influence should be constrained.

All of them agree “ each citizen have the right to get the treatment and just
assume that there is no free market system so how the bore people can get
treatment!

7. Doctors should not accept any gifts from pharmaceutical companies.

4 strongly agree” just assume that these companies gives the doctor any gift in
fact they do not give a gift unless there is a reason and here the reason is to
prescribe their treatment even that it's not the best choice for the patient"

In the end, it's obviously clear that all of the responses were about their own
experience and their own situation as well. These responses it's just indicating how
society think about medical professions and medical stuff. I think there is some
misunderstanding and wrong concept from society about medical stuff. The reason is
usually no one explains the reasons for why make this decision.
2.02 b patient safety

Patient safety is a fundamental aspect of healthcare that aims to prevent and


reduce harm to patients during the provision of medical services. It encompasses a
variety of practices, technologies, and policies designed to protect patients from
errors, accidents, injuries, and infections. Ensuring patient safety is a shared
responsibility among healthcare providers, patients, regulators, and policymakers.

The different types of teams in health-care and the rule for each one of them

1. Core teams
Core teams consist of team leaders and members who are involved in the direct
care of thepatient.

2. Coordinating teams
The coordinating team is the group responsible for:
• Day-to-day operational management;
• Coordination functions;
• Resource management for core teams.

3. Contingency teams
Contingency teams are:
• Formed for emergent or specific events;
• Time-limited events

4. Ancillary services
Ancillary services consist of individuals such as cleaners or domestic staff

5. Support services
Support services consist of individuals who:
• Provide indirect, task-specific services in a health-care facility,

Administration
Administration includes the executive leadership of a unit or facility, and has
24-hour accountability for the overall function and management of the organization.
CASE I (Right action, wrong result)
In this case, there are lots of mistakes that done be the Practitioner. First, when
we have hypertensive patient first thing that we must done is vital signs. Especially,
measuring blood pressure. In this case it wasn't done. Secondly, not enough, and
accurate symptoms. The practitioner does not take any history from the patient And
did not meet him as well!

CASE II (Right action, wrong result)

Working overtime has significantly proven that leads to a lot of mistakes in


work and decrease efficiency of the worker. The junior Nurse administered wrong
dosage amount of medication. The problem is the nurse does not ask or seek help
when she have a doubt for the amount of dosage. Because of difficulty in breathing
and more complications the boy is unfortunately died. We are dealing with human
beings, so we must be careful and sure of every step we take and consult if we have
any doubt about anything.

CASE III (Emergency resuscitation requires teamwork)


lack of awareness of how to deal with emergency cases is something horrible.
especially, from a doctor that he's working in emergency department. The patient was
clearly bleeding and there's injuries everywhere. So, first thing you have to do is it
stop bleeding rather than focusing in something that is less important and it's not
lethal.

CASE IIII (Everyone in the team counts)


As a surgeon, you must ensure the patient safety from medical errors and not
causing harm to the patient. In this case, the surgeon was explaining to the medical
students about the procedure of this surgery. while that he forgetting to remove the
artery clamp. Even if you were explaining and teaching during the surgery, you must
ensure a highly standard precautions to the patients.
