ASSUMPTION OF RISK AND WAIVER OF LIABILITY RELATING TO COVID-19

The novel coronavirus, known as Coronavirus Disease 2019 (COVID-19), which causes
respiratory illness in people, has spread globally, including the United States. COVID-19 is
extremely contagious. The World Health Organization has declared COVID-19 to be a worldwide
pandemic. Campus Hollywood and its Affiliates has implemented various measures to ensure
the health and safety of its employees, students, and any other individuals who enter the campus.
The school is carefully monitoring guidance from local, county, state, federal, and international
government entities such as the City and County of Los Angeles, the State of California, CDC,
OSHA, and the World Health Organization. In addition to following this guidance to reduce the
transmission of COVID-19, we are doing the following:

• Nightly disinfecting of common areas and high touch surfaces.

• Periodic disinfecting of high touch surfaces, restrooms, doorknobs, handrails, etc.
throughout the day.
• Daily self-screening for elevated temperature and COVID-19 symptoms through an
application for employees and students.
• Implementing social distancing guidelines and maximum capacity in classrooms and
other spaces throughout the campus.
• Requiring that all employees and students wear face coverings while on campus.
o Face coverings will be provided at no cost to employees.
• Providing hand sanitizer locations throughout the campus.

Even with the above measures, the company cannot guarantee that employees and
students will not be exposed to COVID-19. You hereby acknowledge that (a) the novel
coronavirus COVID-19 (“COVID-19”) is extremely contagious and is believed to spread mainly
from person-to-person contact, (b) entering the campus and/or attending classes could increase
my risk of being exposed to and/or infected by COVID-19, (c) such exposure or infection may
result in a serious medical condition requiring medical treatment in a hospital or could possibly
lead to demise, and (d) such exposure or infection may result from the actions, omissions, or
negligence of Campus Hollywood and its Affiliates, its officers, directors, managers, shareholders,
agents, employees, volunteers, representatives and/or other Campus Hollywood and its Affiliates
attendees. You, on behalf of yourself, and each of your heirs, successors and assigns, knowingly,
freely and fully assume all such risks. You, on behalf of yourself, and each of your heirs, accept
sole responsibility for any injury to yourself (including, without limitation, personal injury, disability,
demise or other loss or liability) as a result of such risks and/or our entering the campus and/or
attending Campus Hollywood and its Affiliates in connection with COVID-19.

You, on behalf of yourself, and each of your heirs, successors and assigns, hereby forever and
irrevocably release, waive, relinquish, and discharge Campus Hollywood and its Affiliates and its
officers, directors, managers, shareholders, agents, employees, representatives, volunteers and
successors and assigns (collectively, the “Released Parties”) from any and all claims, demands,
liabilities, responsibilities, rights, damages, expenses, and causes of action of whatever kind or
nature, and other losses of any kind, whether known or unknown, foreseen or unforeseen,
specifically limited to COVID-19 (collectively, “Damages”), as a result of my entering the campus
and/or attending Campus Hollywood and its Affiliates. This release includes any Damages related
to any exposure to or infection from COVID-19 (including any personal injury, disability, demise
or other loss or liability related thereto), based on the actions, omissions, or negligence of Campus
Hollywood and its Affiliates or the other Released Parties (collectively, the “Released Claims”).
 By signing below you acknowledge that you have received the opportunity (and been
strongly encouraged) to review this Assumption of Risk and Waiver with an attorney, that you
have carefully read and fully understand the contents of this Assumption of Risk and Waiver, that
you are giving up substantive legal rights, have asked and received answers to all questions you
may have, and that you have not been induced to sign this Assumption of Risk and Waiver by
any promise or representation and sign it freely and voluntarily, intending and agreeing to be fully
bound by the terms hereof.
