Performance Improvement Plan Template

This document outlines a performance improvement plan (PIP) process. It consists of two parts: (1) the PIP form which identifies areas for improvement, expected outcomes, and review dates, and (2) a PIP result evaluation. The PIP is designed to help employees improve performance over 90 days if they received a rating of "2" on their annual review or if performance otherwise falls below standards. Supervisors work with employees to establish specific, measurable goals and provide necessary support resources. Progress is reviewed regularly and consequences are specified if standards are not met by the designated timeframe.
