When is it performed?
Also known as TRIAGE
The protocol for emergency assessments is known as
ABCCS. These assessments include:

➢ Making sure the Airway is not blocked

➢ Ascertaining that the patient is Breathing and
breathing normally
➢ Monitoring Circulation, such as pulse and heart
rhythm
➢ Observing the level of Consciousness and noting any
sudden changes
➢ Ensuring the patient’s Safety from any potential
harms
Two primary Sources of Data

1. Subjective Data
> client’s or patient’s own verbal descriptions of their
health status or problems
> Only client’s provide subjective data
2. Objective Data
> Objective data are observation or measurements of
client health status
